Virtual School Consent Form

I hereby give my consent to the School in collecting, using, and/or disclosing the relevant information about my child and other processes whether automated or manual, for the purpose of and for the fulfillment of my child's virtual school registration for the relevant course my child is enrolled or enrolling.

I understand that the collection of information during registration of my child for the virtual school is needed for the purpose of monitoring the process of my child during the academic period which may include the name of my child, address, age, gender, grade level, course, and my or the designated legal guardian's personal information including phone number.

I understand that my child's personal information may be used and may be disclosed such as student name and image during video transmissions for the purpose of conducting of online learning.

I hereby declare that I am the Parent/Guardian of the aforementioned child and I grant permission for my child to participate in the virtual conduct of the School. I understand that the school shall conduct synchronous learning which is a live interaction between the students and the teacher. Any activity during the conduct of classes cannot be edited before the student witnesses any content. 

I hereby declare that I have had the opportunity to discuss with the School's representative any concerns or clarifications regarding the conduct of carrying out and doing virtual classes.
