Congratulations on your upcoming wedding, and THANK YOU for choosing Sherinne Cosme Artistry to help celebrate your special day!
You are ready to set up your styling and makeup appointments and I am ready to assist you! The purpose of this form is to confirm the services to be rendered for the wedding party along with the name(s) of those responsible for services provided by Sherinne Cosme Artistry. This form must be completed to create schedule and to finalize contract.
You can always edit, add, modify the form via Jotform's confirmation email. After completing the form, please check the confirmation email and save that email in a safe place for easy access whenever you want to add to the form. I must warn you that the form is extremely thorough, so please take the time to fill out the form!
Before you go to complete the form, it is key to have your inspirational and wedding photos already saved in your phone's gallery. This is for easy access when prompted in the form. I would recommend not to browse for photos in the middle of completing this form as the form details may not save. It's okay if you do not have photos yet, but these are great visuals for your wedding hair and makeup goals. (Again, you can add these photos after this form is submitted.)
Lastly, if you have not already have done so, to save your date, a non-refundable deposit of $100 is required. The deposit is applied to your final balance due. If you have any questions at all, please let me know. I will be happy to further assist you!

Special Information for the Bride and her Wedding Party on the Wedding Day
- As part of your service, I provide a timeline/schedule for the morning of to help things run as smoothly as possible! It is important all party members are aware of and ready for their time slot. This helps to ensure we stay on time for the day and ultimately allows for an easier flow for your morning.
- Remember to bring any hair accessories, jewelry, hair extensions, and/or veil to the appointment.
- When necessary, a shampoo will be done at the salon by the stylist (in-salon weddings only & when scheduled).
- Let me know of any and all allergies, skin concerns/issues or anything else that may impact the hair and/or makeup services.
- Please arrive for your appointment with clean, dry, and smooth hair unless otherwise instructed by me.
- For makeup appointments, please have a fresh clean face with light moisturizer on. Please consider bringing your own skincare if you have allergies (eg. moisturizer and a cleanser).
- Remember to wear a button-down shirt or something that can very easily be removed as to not affect the makeup application or hairstyle.
- Remember to clear off the working area (table) and set up near working outlets. A table is needed to layout the hair/makeup kits. Be sure the area is away from food or delicate items. The working area is preferable near natural light (window) or with ample lighting. Thank You!

Preview Schedule
Previews (trials) are not required, but recommended. Previews should be scheduled at least 2 months in advance and are typically done at least 10 weeks prior to wedding. Previews are nonrefundable. (Payment for hair trial is due at the completion of hair preview. Can be cash or card)

When you come in for your preview, I recommend that you bring
• Your headpiece, jewelry, and/or hair extensions
• Pictures of hairstyles and makeup that you like
• A digital camera or mobile phone with camera capabilities
• A picture of your dress and flowers
• Someone whose opinion you value
• It is recommended to wear a white shirt that has a similar neckline to your dress. Be sure you can easily remove your shirt without ruining your hair and makeup (zipper, button-down)
