At this height of the spread of COVID-19, the global economy and the lifestyle of people have been affected. Many countries worldwide imposed lockdown measures in order to mitigate the spread of this global pandemic.

We have put in place policies and preventative measures in order to mitigate the spread of COVID-19 and prevent such infection from happening within the premises; however, there is no guarantee that that as much as we have the preventive measures in place that no one will be infected by COVID-19. 

Before you proceed to this form, please ensure that you have discussed with the facility representative the details that clarify the limitations on which the facility may be answerable. By going through the form this means that you affirm and understand the extent by which the facility may be liable. Please take note that by submitting this form, you agree to the contents of this form by which may affect your legal rights on your continued use of the facilities or participation in any of our crossfit events.

I am aware of the contagious nature of COVID-19 by which exposure and infection may be through being near an infected person. I understand that the risk of being exposed to COVID-19 is very high even if the Crossfit facility has seen to it that preventive measures and policies are set in place.

I voluntarily assume the risk of being exposed to COVID-19 in participation or in attendance to the activities and usage of facilities in the Crossfit facility. I understand that my participation or use of facilities may leave me exposed to or infected by COVID-19 anywhere, including my visit to the Crossfit facility or venue.

I understand and fully acknowledge that by accepting this Agreement, I am waiving certain rights, including the right to recover damages, due to injuries, loss, or death. 

I hereby warrant that I am at least eighteen (18) years of age, that I have read this Agreement in full and carefully understand the terms and conditions relevant hereto. I have had the opportunity to clarify anything ambiguous and have raised my questions to a representative and by which such questions were answered to my satisfaction.
