- Please continue to use the "Mask Order Form" and "Uniform Order Forms" separately.
- Generally, there is a break-in period for Shoes for Crew. You will find them comfortable over time and see that they last about 3 times longer than the slip-resistant work shoes found in local retail stores.
- Chick-fil-A Madison Street does not profit from the sales of shoes, we simply facilitate the ordering as a convenience to you. We make no promises or claims pertaining to the safety, effectiveness, or durability of the shoes.
- If you don't like them for any reason, do not hesitate to order new ones. Shoes for Crews are great at refunding people if not satisfied, within a reasonable time. If the refund is rejected, you will still be responsible for the cost of the shoe. Chick-fil-A Madison Street will not give refunds for shoes, instead, we will extend a credit toward a future shoe, uniform, or mask order.
- It will be assumed that all orders will be payroll deducted over 2-3 paychecks unless otherwise requested.
- Due to applicable labor laws, if you make less than $8 per hour, we require that you pre-pay for all or at least half using cash, personal check, or credit/debit card unless you make other arrangements with Stephanie.
- By submitting this order form, you agree to allow Chick-fil-A Madison Street to payroll deduct the cost of your order in the amount allowed by the State of Tennessee but not so much as to place you under minimum wage on any paychecks.
Pro Tip: At the end of each shift, spray the bottom of your shoes with purple degreaser or floor soap. Then rinse the treading with the hose to release oil and grime built up in the treads. This will extend the life of not only your shoes but your floor mats and home's carpet.
