Consent Form

We would like to ask for your participation in the survey. You will be asked to fill out a questionnaire related to your academic learning in during the academic year. YOUR PARTICIPATION IS NOT RELATED IN ANY WAY TO YOUR GRADE IN THIS SEMESTER. You may also decide to not participate now with no penalty from our institution.  All your responses are strictly confidential and only members of the research team will see your responses. We also ensure that your data will be published only with anonymous identity. Choose NEXT if you agree to participate in this survey.

The following questionnaire consists of 9 parts and each item represent the strategy you use in your academic learning. There are no right or wrong answers. Use the scale (1-7) to answer all questions. If you think the statement is very true of you, choose 7; if the statement is not true of you at all, choose 1. If the statement is more or less true of you, find the number between 1 and 7 that best describes you. You may tap or swipe to the option to choose.

The following questions will ask about how often you use study strategies, such as re-reading class notes and course readings and memorizing lists of key words and concepts.

The following questions will ask about how often you attempt to summarize or paraphrase the material you read in your textbooks, and how often you try to relate the material to what you already know or have learned.

The following questions will ask about your ability to select the main ideas from your readings as well as your attempts to organize and put together what you need to learn in this course.

The following questions will ask about how you ask for other’s help when facing a difficult task or when you don’t know something. They can be classmates, seniors, or lecturers.

We would like you to give us feedback on the questionnaire you have responded to previously. Your response will be useful for the researcher to improve the upcoming study. You will be answering to Yes/No questions and open-ended questions!
