Passive School Counseling Services Opt-out Form

This school year, your child will be offered school counseling services at ASTUMBO ELEMENTARY SCHOOL as part of the student's in- school program. The school counseling services include but not limited to: Academic Counseling College and Career Counseling Personal/Social-Emotional Counseling (Supportive and not therapeutic)

Individual and Small Group Planning Individual and Small Group Supportive Counseling (not therapeutic) Screeners (example includes but not limited to Substance Abuse Screener) Assessments (example includes but not limited to Functional Behavioral Assessment

If you DO NOT want your child to participate in some or all of the counseling services, please put your initial next to the services below, sign the form below and return it to school by [MONTH, DAY, and YEAR]. If we do not receive this form, your child will participate in all counseling services.

School counseling and guidance are very essential in a student's life, and schools have a huge part in bringing out the best in children. School counselors provide a comprehensive program that supports and promotes academic achievement, career and college readiness, and personal/socia development of each student. School counselors provide the following services: counseling and guidance - academic, personal/social, and college and career, classroom guidance curriculum, individual and small group planning, and individual and small group supportive counseling, and conducts screeners and assessments.

Following the updated guidelines from the district, all schools are required to use the Passive School Counseling Consent (Opt-Out The passive consent provides notice that parents and guardians, and/or students may allow school counseling services provided by the department. The use of the consent is effective for SY 2020 - - 2021.School counseling services cannot be provided to a student if the parent or guardian, and/or student "opt-out" (by signing the consent) from the services.

Please complete the attached form and return to the school as soon as possible.
