Medication Evaluation
To be filled out if a client takes Medications
It is important if a client Self Medicates or if Easy iCare is going to be responsible to prompt/remind, assist or administer medications that the correct forms and consents are completed.

No further forms will need to be completed. The Care Manager is responsible to make sure if this changes that they reassess the medication forms required at that time.
Easy iCare workers are required to
Prompt Client to take medications Assist Client to take medications Administer medications to the client
Self Medicating
You will need to complete a Self Medication form to ensure that the client is capable to manage their own Medications. This should be reviewed every 3 months. This can also be done on line (I THINK) on the electronic Self-Administration Form
Easy iCare Managing or involved in Medicating
You will need to complete the Consent form and provide a proper Medication chart on this program that you will have to monitor to ensure we are correctly prompting, assisting or administrating their medications.

A Medication Chart will also need to be implemented on this program and used to show evidence we are prompting, assisting, or administering medications as agreed to.
All staff should complete the online Medication management training and or hold a certificate in Medication Management.
You can choose to use the form below and have staff complete it manually at the program. This form needs to be uploaded by the care manager weekly or monthly for the client file to show evidence this is being done.
You can also have staff electronic Medication Recording form log evidence they prompted, assisted or administered the clients medication. This is advised if the client or staff may be inclined to tamper with the original medication chart
