The PPE Assessment Form can be used to determine the need to use PPE of employees by identifying the hazards involved in the performance of duties by them.
To properly fill out this form:
- Conduct a PPE Hazard Assessment every time when tasks or conditions change to know the effectiveness and need for PPE.
- Make a walkthrough of the work area to see the environment and the tasks of persons in the said area. Identify each and every task to learn and point out the hazards involved for which the employee may be exposed during the performance of duties in the said work area.
- If the hazards cannot be eliminated without using PPE then indicate which type of PPE is needed for the protection of individuals from such hazard.
- Please take note that PPE alone is not an absolute protection against hazards, but should be used in connection with safeguards for good operating practices.
- When selecting a PPE, select the most suitable type available
- It shall be the responsibility of the supervisor to ensure that every employee understands the care and limitations of using PPE.
