Parent/Legal Guardian Agreement for Out-of-Zone Students
I understand that if this transfer is approved by the receiving school, it is granted for one year to be reviewed and renewed annually based on the conditions that my child maintains regular and punctual attendance, good behavior, and complies with the CNMI Public School System and school policies. If these requirements are not met, or if school capacity or class/grade level size exceeds district standards, or other unfavorable conditions develop in the receiving school, I understand that my child's transfer could be denied or revoked. I understand that transportation shall be the sole responsibility of the parent.
Rules and Regulations for Student Transfers
- Transfers are denied or revoked when:
- attendance and/or behavior are poor or
- building capacity is exceeded
- District rules and/or school rules are not met (punctuality, arrival and pick-up times, etc.)
- Class size requires an additional staff member (i.e. teacher aide, additional teacher, or combination class)
- the transfer request is made two (2) weeks within the end of the quarter
- A Zone Waiver Request form must be submitted with this form if the student's residence will be out of zone of the receiving school.
- If the student will be out of zone of the receiving school, the parent/guardian understands and agrees that transportation will be the sole responsibility of the parent.
- Both 5th and 8th grade students must reapply for transfer if they plan to attend a different middle/high school other than their designated receiving (rollover) school.
- For SY 2020-2021 only, students who are "out of zone", but have been enrolled at their current school since last school year, do not need to turn in a Zone Waiver Request.
- Any due obligations a student may have incurred at the withdrawing school will be transferred to the receiving school. Requested copies of transcripts and report cards will not be certified as "official" documents until all obligations have been cleared.
- A Parent/Guardian of the student must complete a copy of this form.
- Only 1 transfer is granted per semester per student.
