HIGH-RISK DEFICIENCIES: AREA- BRIDGE
"- PASSAGE PLAN NOT IN ACCORDANCE WITH SOLAS V Reg.34. (i.e.
No consideration of Marine Environmental Protection. GBRMP (PSSA) & No REEF VTS pre-entry report).
- ECDIS alarm not set properly.
- Selection of the Correct Navarea (pertaining to Australia) in the Inmarsat-C, EGC Set Up. Original BA/ Aus. Charts NOT on Board.
- Documentation (Bridge Checklists / Engine + Deck Log + ORB + GRB, etc.) not in order. Rudder Angle Indicator not returning to Zero when wheel amidships.
- MF/HF DSC self-test fails / no acknowledgment from shore stations/staff not familiar with operations.
- Change over from AC to DC not known by deck officers. Voltage drop very drastic, cannot hold a charge"

HIGH-RISK DEFICIENCIES: AREA-SURVIVAL CRAFTS
- Lifeboat engine failed to start in three consecutive starts.
- Lifeboat On-Load Release Hooks not correctly set/reset. Lifeboat not lowered/maneuvered as per SOLAS.
- Crew not “fully” aware of the launching /recovery operations of the lifeboat.
- Hydrostatic interlock not correctly re-set or mechanism not understood by the crew.

HIGH-RISK DEFICIENCIES: AREA- FIRE FIGHTING SYSTEMS
• FFA equipment’s not in readiness/staff not familiar with their operation/servicing requirements.
• Fire Line Hole/thinned down, Fire Hoses and hydrants leaking.
• EEBD’s Empty.
• Hyper Mist System not in readiness.
• Co2 fixed firefighting system not in state of readiness.

HIGH-RISK DEFICIENCIES: AREA-MARPOL
- Chief Engineer & 2nd Engineer not familiar with the operation of OWS and Incinerator.
- Oily Water Separator Defective and crew unable to demonstrate operation.
- Garbage Record Book – Entries not as required / Cargo residues entries not completed.
- Sewage treatment plant defective.

HIGH-RISK DEFICIENCIES: AREA – ENGINE ROOM
- Setting of 220V & 440 V Feeder panel in engine room NOT made correctly and provision for testing the alarm is NOT provided.
- Funnel Flap not closing + Fire dampers not sealing / wasted or otherwise weak flaps.
- Quick Closing Valves not operating correctly or not operating at all.

HIGH-RISK DEFICIENCIES: AREA-MLC
MCV is not available for all crew. The vessel was fined 5000 AUS$.
Crew certification NOT in compliance with MLC 2006 (e.g – Crews tenure on board beyond the Contract period).

HIGH-RISK DEFICIENCIES: AREA-MISECELLANEOUS
• Annual Ballast Pump Test Requirement not carried out.
• Ballast Tank Air pipe vent Head / Float condition poor, stuck, missing or damaged.
• Chemical Storage unsatisfactory, no proper segregation, Incomplete MSDS.
• Accommodation Flaps Seized / Hole in Funnel Body.
• Accommodation / Housekeeping – Not in order. Toilet Flush not Working
/ Common Toilets Dirty.
• Structural Deficiency (Cracks, etc).
• Helicopter Operations (Deck Strengthening Certificate from Class or Yard / FFA Equipment & Tools for the helicopter operations).

• Shippers declaration on board

In order to prepare with requisite documents at hand while the PSC inspection is going on please see the below list. Kindly note the documents are complete, no missing pages, parts, and where required, updated to date.
The list is not exhaustive but is general guidance so we are prepared as much as possible.
It would be helpful if the following documentation could be assembled together in one location for inspection:

• Ships Certificates (Trading and Tertiary Certificates)
• Last PSC inspection reports and their closing reports/evidence.
• Officers & Crew Certificates of Competency, STCW endorsements and Certificates of Recognition
• Appropriate Cargo Publications and Loading plan
• Approved stability information
• Class Survey report file (including Enhanced Survey report file where required)
• Records of drills (official log book)
• Garbage management plan and record book
• Oil record book
• SOPEP/ SMPEP (Chem Tankers)
• SEEMP
• P&I Insurance (Certificate of Entry)
• Shore based radio equipment maintenance agreement
• Cargo gear record book
• Cargo securing manual
• Document of Compliance (Dangerous Goods)
• Internal/ External Audits, Flag Audits etc and their closings.
• Master review of ISM System and Company’s response to same.
• Near Miss, Risk Assessment, Safety Meeting Files and their officer responses/ closings.
• Annual test certificate (or evidence of testing) of 406 EPIRB
• P And A Manual (for chemical Ships only)
• Documented Emergency Procedures. Damage Stability Booklet

I may also require to operationally test the following items of equipment:
• Engine room fire dampers, funnel flaps
• emergency fire pump
• emergency generator (no black-out), Simulation test. Interview for the operation to take on load.
• emergency steering system
• oily water separator (no discharge overboard)
• Fuel oil line high pressure leak alarm
• Engine Room, Pump Room (if applicable), Fore Castle bilges alarms to be tested.
• Steering Gear Hydraulic Oil low level and other failure alarms
• Boiler Safeties (Low water level, Flame failure etc)
• Lifeboat engines
