Wedding Consultation Form
You are ready to set up your styling appointments and I am ready to assist you! The purpose of this form is to confirm the services to be rendered for the wedding party along with the important wedding day details. This form must be completed to create schedule and to finalize contract.
Before you submit the form, it is key to have all inspirational photos for bride and wedding party.
Lastly, a non-refundable deposit of $100 is required. The deposit is applied to your final balance due. If you have any questions at all, please let me know. I will be happy to further assist you!

Special Information for the Bride and her Wedding Party on the Wedding Day
- As part of your service, I provide a timeline/schedule for the morning of to help things run as smoothly as possible! It is important all party members are aware of and ready for their time slot. This helps to ensure we stay on time for the day and ultimately allows for an easier flow for your morning.
- Remember to bring any hair accessories, jewelry, hair extensions, and/or veil to the appointment.
- Let me know of any and all allergies, skin concerns/issues or anything else that may impact the hair services.
- Please arrive for your appointment with clean dry hair unless being prepped for style in salon.
- Remember to wear a button-down shirt or something that can very easily be removed as to not affect the hairstyle.
- Remember to clear off the working area (table) and set up near working outlets. A table is needed to layout the hair kit. Be sure the area is away from food or delicate items. The working area is preferable near natural light (window) or with ample lighting. Thank You!

Preview Schedule
Previews (trials) are not required, but recommended. Previews should be scheduled at least 2 months in advance and are typically done at least 4 weeks prior to wedding. One complementary preview is included in price.
When you come in for your preview, I recommend that you bring
• Your headpiece, jewelry, and/or hair extensions
• Pictures of hairstyles that you like
• A picture of your dress
