Falkinham Scholarship Application

About the Falkinham Scholarship: 

Ken and Sandy Falkinham live in Wadsworth, IL.  Ken was CFO of three Illinois school districts for 22 years, including 5 years for Waukegan SD 60 and he also had a successful career in energy companies that arose as electricity/natural gas was deregulated. Sandy was a kindergarten/first grade teacher for 33 years, 21 of them with Waukegan SD 60. The Falkinhams have a deep Christian faith and passion for education. 

The Falkinham Scholarship is a 4-year scholarship* of up to $10,000 per year, based on need, to be awarded to the W2C senior who has demonstrated:

·      Financial need (Gap based on financial award letter)

·      Demonstrated leadership in your community (e.g., school, church, home)

·      Participation in W2C 

*Recipient must maintain at least a 2.5 cumulative GPA while in college to retain the scholarship

Essay #1 - How do you anticipate your education will help you make a difference in your community?  **500-word max

Essay #2 - Describe why you need this scholarship.  **300-word max

Essay #3 - Describe the aspects of W2C (e.g., expanding horizons, mentor, scholarship club, college application assistance, workshops, community service, tutoring, college visits) that have been the most beneficial on your path to college. **100 word max

List all the scholarships you have applied to, the amounts, and indicate the status of each:

CERTIFICATION

 In submitting this application, I hereby certify the following:

1.     I am in need of this scholarship aid as set forth in this application. 

2.     I understand that W2C will pay the scholarship funds directly to my educational institution and that only tuition for credit courses and/or campus housing will be paid. 

3.     I agree to provide W2C with a copy of my tuition bill with my student ID/account number and my transcript at the beginning of each scholarship term. 

4.     I understand that this scholarship will not be retained if my cumulative GPA falls below 2.5.

5.     I hereby certify that all of the information I have provided within the entire scholarship application is true and correct to the best of my knowledge. I understand that if any information submitted with the scholarship application is determined to be false or mislead­ing (including any omission of material information), I will be disqualified from consideration for the scholarship.

Finally, I give consent to allow W2C use a photograph (or other likeness), as well as personal information about me (family, name, hometown, etc.), or statements for publicity purposes, which may include, but is not limited to, publication in newspapers, brochures, catalogs and the W2C website.
