hereby consent to Gratis Recruitment & Gratis Home Care obtaining any personal information either by:
- Directly from you, when you provide information by phone or in documents such as an application form or a resume;
- Contacting the referees that you have nominated;
- Clients to whom services are supplied as part of our contracting business; and by searching publicly available information (for example, the internet).
- any government authority or department including but not limited to: The Police, any information and/or documentation concerning any Apprehended Violence Orders made against me and any criminal charges or convictions against me which in any way related to:
- A criminal offence involving any breach of licensing legislation in Australia; or
- A criminal offence involving dishonesty; or
- A criminal offence involving violence; or
- A criminal offence involving harassment; or
- A criminal offence of a sexual nature; or
- A criminal offence involving drugs.
I authorise Gratis Recruitment & Gratis Home Care to provide a copy of this to the relevant authority with any request by it for information.
I understand the reason this information is required by Gratis Recruitment & Gratis Home Care is so that Gratis Recruitment & Gratis Home Care can satisfy itself that I am a fit and proper person to be employed by their clients. I acknowledge Gratis Recruitment undertaking to me that any information and/or documentation so obtained by Gratis Recruitment will not be used for any purpose other than assessing my suitability for employment. If I am unsuccessful in obtaining employment through Gratis Recruitment, I note Gratis Recruitment’s & Gratis Home Care’s undertaking to me that all such information and/or documentation will be promptly destroyed if requested or else it will be kept on file for any future applications.
Access to your personal information
You may access your information at any time (subject to some exceptions allowed by law). If you would like to know what information we hold about you, please contact our Privacy Officer. For security reasons, you may be required to put your request in writing. Further, you may be charged a fee for searching for and providing access to your information.

Some examples of the types of personal information that may be held by Gratis Recruitment & Gratis Home Care includes your name, date of birth, contact details, qualifications, work history, skills, resume, salary/benefits information, driver's licence number and information about your hobbies and interests. If you consent, we may also collect sensitive information (for example, information about your membership in a trade union or health information). It is up to you to choose the information that you give to Gratis Recruitment & Gratis Home Care. If you choose not to provide Gratis Recruitment & Gratis Home Care with information that has been requested, Gratis Recruitment & Gratis Home Care may not be best placed to provide the service that you have requested.
