This is an individual form by student; a family with multiple students needs to fill out a separate form for each student. Please complete this form by Friday, March 26, 2021, at 4:30 pm*.
If there are siblings in your family planning to enroll, please use the open enrollment process posted on the school website to enroll for the 21-22 school year.
