Good Samaritan Hospital is happy to provide additional augmented discharge planning services for our patients through a partnership with Kern Health Systems and the State of California.
This additional discharge planning and assistance is provided to you free of charge.
The services that we are able to offer include:
1. Additional discharge assistance
2. Assistance in getting to your first appointment after discharge
3. Assistance in obtaining your medications after discharge
4. Coordinating with your outpatient provider to ensure a smooth transition to your appointment.
Enrollment in this new program is optional and you may opt out at anytime.
In order to provide these services, your discharge planner and social work team may need additional information about your location, housing, outpatient appointment time and backup contact information.
