KCS is offering Pfizer vaccinations for friends and family members of KCS employees on April 1, 2021 who are low-income and have limited English proficiency. KCS would like to prioritize individuals whose primary language is Korean, Spanish, or another Asian language. KCS will vaccinate the following in the current tier following County guidelines:
- Phase 1A
- Phase 1B
- Ages 65+
- Ages 16-64 with Medical Conditions**
- 50 and Older
Each employee is limited to four referrals who meet the criteria mentioned above. Please submit the form for each person you are referring and note their appointment time. Each person will be limited to four submissions. Do not resubmit if an incorrect submission was made. Instead, please email yukim@kcshealthcenter.org and jakim@kcshealthcenter.org with the correction to be made.
Deadline for submission is by Friday, March 26th by 5 PM
Please be advised that the following are required when getting vaccinated:
- Identification - Picture ID/Name that matches appointment
- Insurance card (if applicable)
- Documentation proof (i.e. paystub, letter from employer, badge) or Self-Attestation for Eligible category

451 W. Lincoln Ave. Anaheim, CA 92805
To speed up the registration process, please submit a photocopy of the individual's ID card and insurance card.

**Qualifying medical conditions for vaccination:
- Cancer, current with debilitated or immunocompromised state
- Chronic kidney disease, stage 4 or above
- Chronic pulmonary disease, oxygen dependent
- Down syndrome
- Immunocompromised state (weakened immune system) from solid organ transplant
- Pregnancy
- Sickle cell disease
- Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies (excludes hypertension)
- Severe obesity (Body Mass Index ≥ 40 kg/m2)
- Type 2 diabetes mellitus with hemoglobin A1c level greater than 7.5%
OR
If as a result of a developmental or other severe high-risk disability one or more of the following applies:
- The individual is likely to develop severe life-threatening illness or
death from COVID-19 infection - Acquiring COVID-19 will limit the individual's ability to receive ongoing care or services vital to their well-being and survival
- Providing adequate and timely COVID care will be particularly challenging as a result of the individual's disability
