I, blanks*hereby acknowledge my awareness that my participation in the physical activities, sponsored during Savannah State University’s Campus Activities Board (CAB) Field Day, such as the Bounce It Out Events, can involve MANY RISKS OF INJURY including, but not limited to property damage, personal injury such as broken bones and emotional trauma, and serious bodily injury, where I may need to be hospitalized, or death.
I knowingly, freely and voluntarily assume all risks and engage myself in the participation of the above mentioned activities.
In consideration of participation in Campus Activities Board (CAB) Field Day activities, I hereby release and hold harmless the Board of Regents of the University System of Georgia, Savannah State University and the Campus Activities Board (CAB), its employees and volunteers, from any and all liability arising out of my participation in the above -mentioned activities, and hereby waive my rights herein to assert any claim(s) for damages, bodily injury or serious bodily injury to the fullest extent allowed by law arising out of or in any way related to the activities.
I understand and agree to accept and abide by all the rules and requirements of the activities, including safety rules and instructions given by the supervisory personnel.
This Agreement shall inure to the benefit of and be binding upon my heirs, decedents, successors, executors, assignees, legal representatives, and all family members. The provisions of this agreement including, but not limited to, my waiver of liability and my assumption of risk shall survive this agreement:
