Due to the unknown duration of care, and the need of the Caregiver to know when the foster arrangement will be concluded, Custodian agrees that the Caregiver will not be responsible for keeping the animal any longer than the duration shown:

Custodian Acknowledgement

Custodian is unable to care for said animal; and Custodian desires the Caregiver to hold said animal for Custodian in Custodian's absence until said animal can be reclaimed.
Custodian recognizes that the Caregiver cannot hold said animal for Custodian for an indefinite period of time. In consideration of the premises and the covenants herein contained,it is agreed between the Caregiver and Custodian as follows:
If the Custodian does not return for the animal within said period or if the Custodian states to the Caregiver that they no longer desire return of said animal, the Custodian relinquishes any claim, right, title, or interest in said animal.
If the Custodian does not return for said animal within the set time period, said animal becomes the property of the Caregiver and the Caregiver may exercise complete discretion as to what to do with the animal.
If said animal dies or becomes accidentally injured, ill, or lost, Custodian does not hold the caregiver responsible and releases the Caregiver from any claim arising from said injury.
If said animal bites or injures any human or other animal, Custodian will hold harmless and indemnify, and protect the Caregiver, from any claim or suit filed by anyone as a result of such an incident.
Custodian acknowledges that the Caregiver reserves the right to seek veterinary care without approval by the Custodian (at the Caregiver's expense).
Custodian acknowledges that the Caregiver reserves the right to euthanize the animal Only if a veterinarian deems the animal's health to be so impaired that to sustain the animal would be inhumane.
Custodian acknowledges that upon acceptance by both parties, this is a binding contract, enforceable by civil law.
Custodian hereby delivers to and deposits with the Caregiver said animal.

Caregiver Acknowledgement

Caregiver agrees to notify Custodian of any change of address or phone number immediately.
Caregiver agrees to care for said animal for the duration of time set forth by the Custodian.
Caregiver agrees not to sell, trade or dispose of said animal.
Caregiver agrees that said animal will not be sold or given to a pet store, supplier or wholesaler.
If at any time the Caregiver is unable or unwilling to care for said animal, Caregiver must immediately notify the Custodian & allow 48hrs for the Custodian to reclaim said animal before surrend to an animal shelter is made.
Caregiver agrees to adhere to all state and local animal laws and agrees that said animal will not be used for any illegal purposes.
Caregiver agrees that said animal will NEVER be fought in any way.
Caregiver agrees that said animal shall not be used for medical or any other experimental purposes.
Caregiver agrees to care for said animal humanely (including providing adequate food, water, shelter, and attention).
Caregiver acknowledges that the Caregiver reserves the right to euthanize the animal Only if a veterinarian deems the animal's health to be so impaired that to sustain the animal would be inhumane.
Caregiver agrees not to alter in any way the appearance of said animal without written permission of the Custodian. This includes declawing and cropping of ears/tails.
If necessary, Caregiver agrees to have said animal vaccinated with necessary inoculations; and vaccinated against Rabies, as local ordinances requires (at Custodian's expense).
The Caregiver acknowledges their right to seek veterinary care without approval by the Custodian (at the Caregiver's expense).
Caregiver agrees to provide veterinary records pertaining to any care given while said animal is in foster care to the Custodian as soon as possible.
Caregiver agrees that said animal shall be maintained in an adequate enclosed area or on leash at all times. Animal shall not be allowed to roam at will.
Caregiver agrees to notify the Custodian immediately if said dog becomes lost or stolen.
Caregiver must report periodically as to the well being of said animal and must allow a representative of the Custodian to inspect the animal and premises where the animal is kept. (Such inspections may be random and without notice.)
Caregiver understands the above terms & conditions and understands a breech of contract may result in confiscation of said animal.
Caregiver acknowledges that upon acceptance by both parties, this is a binding contract, enforceable by civil law.

This Agreement is the entire agreement of the parties, and there are no oral promises or representations made in addition to this contract and it may only be changed by both the Caregiver and Custodian.
