  • Daily Vehicle Inspection Report

    This form is to be completed at the commencement of each shift, PRIOR to the vehicle being driven.

  • Is the vehicle in a clean state and free from personal items, rubbish and equipment?
  • Is there any new damage on the vehicle?
  • Has the vehicle been refuelled?
  • Are all the strops complete and on hand (van = 4 small, truck = minimum 6 large)?
  • Are the vehicle lights in good working order?
  • Are the vehicle fluid levels in good order? (check water, oil and screen washer)
  • Are tyres in good condition? (check pressures, tread and side walls)
  • Are safety chain/s in good condition and fitted correctly?
  • Are there any obvious signs of wear on the coupling, tow ball, chain, shackles or any other areas?

  • I hereby acknowledge that I have provided full and accurate information, and accept the terms and conditions of the company vehicle policy. I understand that I am obligated to report any vehicle damage that occurs whilst I am in control of any company vehicle. I also understand that I must adhere to all road rules, and that infringement notices received whilst I am in control of a company vehicle are my responsibility. Additionally, I understand that I may be required to reimburse the company for the cost of any vehicle repairs or insurance excesses that are incurred by my actions.
