    All On Festival attendees are required to read, sign and submit prior to your arrival on 24 June, 2021. No attendees will be permitted to attend ON Festival without this document submitted.

  • Health Check Questions

  • Please confirm the following:

    • You are not suffering from any coronavirus symptoms and have not had any symptoms within the previous 7 days.
    • You have not (as far as you are aware) been in contact with anyone with coronavirus symptoms within the previous 14 days.
    • You are not ‘shielded’ or have any underlying health conditions which might make you particularly vulnerable to coronavirus. 
    • You undertake to declare immediately to your Freshworks contact by phone or email any onset of symptoms or contact with anyone who has symptoms of coronavirus - Please note personal data will not be shared.
  • I confirm that I do not have any of the above listed symptoms and that I agree to declare immediately if my situation should change.*

  • Individual Responsibility

  • Everybody attending On Festival has an individual responsibility to prevent transmission of COVID-19 by following our venue COVID-19 Preventative Measures and doing the following:

    • Maintain a minimum 2m distance at all times wherever possible.
    • Wear a facemask at all times whilst on site. 
    • No handshakes, no kissing, no hugging.
    • Wash your hands frequently or sanitise your hands with alcohol-based
      gel.
    • Follow Catch It, Bin It, Kill It where possible or cough or sneeze into your sleeve, not your hands.
    • Avoid touching face, eyes, nose or mouth with unclean hands.
    • Be diligent with cleaning – both at home and in the workplace,clean your equipment and surfaces.
    • Please follow all instructional signage on flows throughout our building.
    • Keep your distance and ask others to be considerate PPE.
       
      Please ensure you bring your own face mask, as you will be required to wear this at all times whilst moving around site.

  • Cleaning

    Hand sanitiser is available as soon as you arrive at the venue. Please make sure that you frequently sanitise your hands throughout the day. 

  • Social Distancing

    Please maintain a 2m distance at all times wherever possible. Please wear a mask at all times. Keep your distance and ask others to be considerate

  • Acknowledgement

    By signing and submitting this declaration I acknowledge the contagious and evolving nature of COVID-19 and voluntarily assume the risk that I may be exposed to the virus and become affected as a result. I understand the risk of becoming infected may impact members of the community, event guests, and event attendee’s families or personal contacts. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any expense, liability, illness, injury, disability or death related to contracting the virus while attending ON Festival. ​I hereby release, covenant not to sue, discharge, and hold harmless Freshworks, Inc. and/or GroundSwell Group from all claims of any kind arising out of COVID-19.
