This form is used for customers under the age of 18 who play laser tag WITHOUT their parent or legal guardian. This form allows the parent/guardian responsible for the customer under the age of 18 to review the terms and conditions and provide approval by signing.

Informed Consent and Acknowledgement
I confirm and agree that the player is physically fit and mentally able to take the strain and exertion involved in playing Laser Tag games. Has no medical condition that would prevent them from playing. (Please make the game manager aware if they use an inhaler or any other medical aid). Is fully aware of the risk to themselves and others involved in playing Laser Tag games. Has read and will comply with the rules, use all equipment as instructed and not so as to injure or hurt others and obey all instructions from the game manager/site marshals. Will look after the equipment in a responsible way and return it all at the end of the session, and report any damage.
