Mathematics Tutoring Application

The information you provide on your application will be included on a spreadsheet that will be shared with area parents / students who have inquired about tutoring help.

Please agree to the following before completing this application:

If contacted to tutor, regardless of your availability, you will respond within 24 hours.
You will arrive on time and prepared for tutoring appointments
You will keep all scheduled appointments. If you need to cancel, you agree to call the tutoree and offer to try to find a replacement for that session. Assistance with possible names and contact information of other tutors can be gained by contacting the Math Department office.
You will notify the Math Department office if you are not able to take on any tutoring jobs or if you decide to opt out of the pool.
If a request for tutoring makes you uncomfortable in any way, please do not accept the job and report the circumstances to Mary Erickson.
Always meet in a publc place - no tutoring is allowed in student's homes.
