Accent Coaching Agreement Form

This Accent Coaching Agreement (“Agreement”) is being made between Nicole Parker of Parker Accentuation (that’s “Nicole”) and {fullName} (that's "The Client"). Effective date: {dateYou}ServicesNicole agrees to perform the following services, based on the Compton P-ESL Accent Modification Method, to be completed within 90 days of the effective date of this contract: One (1) Initial assessment; One (1) Introductory Session, to review the Initial assessment; Six (6) online coaching sessions, each covering up to 4 phoneme targets; One (1) practice plan per session Feedback from Nicole One (1) Final assessment, with recommendations for continuing practice. In order to get the most out of the accent coaching, the Client agrees to: at least twenty (20) minutes of practice daily as is possible (we know life happens!), using the practice plans outlined by Nicole (if you can, do an hour per day in 10-15 minute segments!). Voice recording as requested (this will NEVER be shared with any entity except Nicole and the Client unless mutually agreed upon).CompensationThe Client agrees to pay the agreed-upon amount, as per the invoice sent to them by Nicole.ExpensesAny expenses incurred by Nicole are the responsibility of Nicole. Any expenses incurred by the Client are the responsibility of The Client.Cancellation/rescheduling policyAny cancellations will be made with at minimum 24 hours’ notice. If the Client does not show up for the session with no notice, or cancels with less than 24 hours’ notice, that session is lost and no make-up will be given. Any cancelled sessions will be rescheduled to within 2 weeks of the date originally scheduled, otherwise they are forfeit. If technical difficulties prevent a session from being conducted or continuing, the session will be rescheduled to within 2 weeks of the date originally scheduled.Refund policyTermination of this Agreement by The Client If The Client chooses to terminate this coaching agreement BEFORE the Initial Assessment has been recorded, The Client will be fully refunded for any amount paid for the coaching agreement. If The Client chooses to terminate this coaching agreement AFTER the Initial Assessment has been recorded, but before the second (2nd) coaching session, The Client will be refunded 50% of the amount paid for the coaching agreement.NO REFUND is due if The Client chooses to terminate this coaching agreement anytime AFTER the Initial Assessment has been recorded and the second (2nd) coaching session completed. Termination of this Agreement by Nicole If Nicole chooses to terminate this coaching agreement BEFORE the Initial Assessment has been recorded, The Client will be fully refunded for any amount paid for the coaching agreement. If Nicole chooses to terminate this coaching agreement anytime AFTER the Initial Assessment has been recorded, The Client will be refunded 50% of the amount paid for the coaching agreement.LocationAll services will be conducted via Internet, unless in-person service was previously agreed upon.Technical difficulties If technical difficulties prevent a session from being conducted or continuing, the session will be rescheduled to within 2 weeks of the date originally scheduled.NO GUARANTEE of improvementNeither Nicole or anything within this contract or any conversation between the Client and Nicole will be construed as a guarantee of improvement. Based on the research that was conducted during the development of the PESL method, individuals who completed the program, including all practice assignments, displayed improvements of 50 percent or greater in post-training assessments, but this is NOT guaranteed.TermThe term of this Agreement is Ninety (90) days from the Effective Date, unless terminated earlier by either Nicole or the Client.TerminationIf either Party wants to terminate the Agreement before the termination date, they must provide the other Party a Seven (7) day written or email notice. This Agreement will automatically terminate when both Parties have performed all of their obligations under the Agreement.

All parties understand and agree to all that is above. Client(s) agree(s) to only sign if they have read and agreed to the terms in this contract and they believe all information is deemed correct to the best of their knowledge.
