Mentorship Agreement Form

I request the assistance of "The Mentor" in helping me build a successful business. The following statements summarise and outline our working relationship that will initiate from the signing of this document.

I will pay a monthly fee to The Mentor whilst The Mentor agrees to mentor I and help them build a successful business. I will pay The Mentor in advance of the first day of each month. The Mentor reserves the right to suspend mentorship and retract all intellectual property if the invoice is not paid on time. I will give The Mentor no less than one month’s notice for termination of this agreement. I may give The Mentor no less than one month’s notice for suspension of this contract. I may suspend for one month throughout the term of the contract. I will pay as usual for the upcoming month but will not pay in the month in which I return from suspension. I understand The Mentor is entitled to four weeks absence every calendar year. I will provide 1:1 or group services to any of my clients. I am responsible for collecting all income generated from services provided. I should arrange to collect all fees directly from my clients. I agree that The Mentor is not liable for any unpaid sessions that may arise. As a self-employed professional I will be responsible for my own tax deductions and national insurance contributions, although advice on how to do this will be provided. I must have and maintain my own Public Liability Insurance to provide cover to $X million. I must hold an up to date first aid certificate and am responsible for arranging this. I must have a current and recognised qualification that permits me to provide the services I offer legally.

By working with The Mentor, I agree that full rights to the use of content resulting from photography/video filming during meetings/ sessions, and any reproductions or adaptations of the images can be used for fundraising, publicity or other purposes to help achieve the aims and objectives of The Mentor. This might include (but is not limited to), the right to use them in printed and online publicity, social media, press releases and funding applications. I consent to The Mentor photographing and/or filming me and for these images or videos to be used for marketing and other promotional uses, including (but not limited to) it’s use on its associated websites and social media channels.
