In my capacity as Pilot-in-command (PIC) of this flight, I acknowledge that I have read, understood and shall conduct this flight in compliance with the regulations, instructions and information referred to below. I confirm that I comply with all the applicable aircrew licensing regulations to enable me to act as PIC on this flight and will adhere to flight and duty periods.I have read and understood COVID-19 Procedure and I commit to ensuring all procedures are followed. I hereby certify that I have sanitized the aircraft prior to flight as per protocol outlined and will ensure all passengers and crew are briefed on the procedure and health screened and passenger documents completed.

8. Fuel Calculations in compliance with CAR’s, CATS and the Operations Manual ( CAR 91.07; SA-CATS OPS 127.07.10; SA-CATS-OPS 127.04.2)
9. NOTAMS pertinent to the route of the flight
10. MET Reports and VFR Minima
11. Current Safety Notices and Tags
12. Current and valid Certificate of Release to Service and any other documentation applicable to this flight.
Pilot’s post-flight signature indicates that:
I confirm that all the necessary post-flight documentation has been completed and al equipment has been retained on board or is securely stored inside the ACME Company Service offices. All Maintenance discrepancies that were noted during this flight have been recorded in the aircraft flight folio and reported to the appropriate maintenance engineer/organization and the Responsible person Aircraft.I hereby confirm that I have sanitized the aircraft post-flight in line with COVID19.
