I, participant undersigned, hereby grant permission to [Production Company] ("Producer") to use my likeness, voice, and performance in connection with the above-mentioned production.
Description of Participation:
I understand and agree that I am participating as an actor/performer in the production described above. I acknowledge that my performance may be recorded, filmed, and/or photographed.
Rights Granted:
I grant the Producer the worldwide, non-exclusive, and irrevocable right to use my name, likeness, voice, and performance in the production and for promotional and marketing purposes related to the production. This includes, but is not limited to, the right to use and reproduce my performance in advertising, trailers, posters, social media, and all other promotional materials.
Compensation:
I acknowledge that I may or may not receive compensation for my participation in the production, as determined by the terms and conditions agreed upon separately.
Release and Waiver:
In consideration for my participation in the production, I hereby release and discharge the Producer, its agents, employees, and all third parties acting under its authority, from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or in equity, whether known or unknown, arising out of or in connection with the use of my likeness, voice, and performance.
Confidentiality:
I agree not to disclose any confidential information about the production that I may become privy to during my participation.
Representation:
I represent that I am of legal age and have the full right and authority to enter into this release.
Governing Law:
This release shall be governed by and construed in accordance with the laws of [State/Country].
Entire Agreement:
This release constitutes the entire agreement between the parties concerning the subject matter hereof and supersedes all prior or contemporaneous agreements, understandings, and communications, whether oral or written.
