Facility Management Waiver Statement
I, the undersigned, acknowledge that I am voluntarily accessing and/or using the facilities, equipment, and services provided by Facility. I understand that the use of the Facility may involve certain risks, including but not limited to accidents, equipment malfunction, property damage, or personal injury.
By signing this waiver, I hereby voluntarily assume all risks, known and unknown, associated with my presence in and use of the Facility, including any activities, programs, or services conducted therein. I release and hold harmless Facility, its owners, managers, employees, contractors, volunteers, and affiliates from any and all liability, claims, or demands that may arise from my use of the Facility, except in cases of gross negligence or intentional misconduct.
I agree to indemnify and defend Facility against any claims, damages, or expenses resulting from my actions, conduct, or use of the Facility. I agree to follow all posted rules, regulations, and instructions provided by Facility staff, and understand that failure to comply may result in suspension or revocation of access privileges without refund or compensation.
I confirm that I am physically capable of using the Facility and its equipment, and I accept full responsibility for any medical care or expenses that may arise due to my participation or presence in the Facility.
