Risk Management Waiver Statement
I, the undersigned participant, acknowledge that I am voluntarily choosing to participate in the activity, program, or event provided by the Organization. I understand that participation in the activity involves certain inherent risks, hazards, and dangers that could result in personal injury, illness, property damage, or, in rare circumstances, serious harm.
By signing this waiver, I hereby:
- Assumption of Risk: I acknowledge and willingly accept all known and unknown risks associated with my participation in the Activity.
- Release of Liability: I release, discharge, and hold harmless Organization, its owners, directors, employees, contractors, volunteers, and affiliates from any and all claims, demands, or causes of action arising out of or in connection with my participation in the Activity, except in cases of gross negligence or intentional misconduct.
- Indemnification: I agree to indemnify and defend Organization against any claims or expenses (including legal fees) that may result from my actions or participation in the Activity.
- Medical Authorization: In the event of injury or illness, I consent to receiving necessary medical care and accept full responsibility for any related expenses.
Acknowledgment: I confirm that I am physically capable of participating in the Activity and have disclosed any relevant health conditions to Organization. I have read, understood, and voluntarily agree to the terms of this waiver.
