I, the undersigned, acknowledge that I am voluntarily participating in sports, fitness, or recreational activities (“Activities”) organized, managed, or overseen by Club. I understand that participation in such Activities may involve inherent risks, including but not limited to slips, falls, collisions, equipment failure, overexertion, or other accidents that could result in personal injury, illness, property damage, or, in rare cases, serious harm.
By signing this waiver, I hereby:
- Accept and assume all known and unknown risks associated with my participation in the Activities, recognizing that no amount of supervision or safety management can eliminate all potential hazards.
- Release, discharge, and hold harmless Club, its coaches, managers, employees, volunteers, affiliates, and sponsors from any liability, claims, or demands arising from my participation in the Activities, except in cases of proven gross negligence or willful misconduct.
- Agree to indemnify and defend Club against any claims, damages, or expenses (including attorney’s fees) resulting from my actions or non-compliance with rules and instructions.
- In the event of an injury or medical emergency, I consent to receiving appropriate medical treatment and accept full responsibility for any associated costs.
- Agree to follow all safety guidelines, rules, and instructions provided by Club, and understand that failure to comply may result in suspension or removal from participation.
