Broomball Participation Waiver
By signing this waiver, I acknowledge that broomball is a physical sport played on ice and involves inherent risks, including but not limited to slips, falls, collisions with other participants, contact with equipment, and other potential hazards that may result in injury.
I understand that participation in this event is voluntary, and I willingly assume all risks associated with taking part in broomball activities during the event.
I hereby release, waive, and discharge the event organizers, hosts, volunteers, sponsors, venue operators, and affiliated organizations from any and all liability, claims, demands, or causes of action that may arise from my participation in this event, including any injury, loss, or damage.
I confirm that I am physically capable of participating in this activity and will follow all rules, safety instructions, and directions provided by event staff and officials.
