Mutt School Pre-Consult Questionnaire

Thank you for choosing Mutt School to help with your dog!

We are looking forward to meeting you soon at your first consultation. Please be sure to complete and submit this questionnaire at least 24 hours before your scheduled consultation.

Behavior problems include fear, anxiety, aggression, excessive barking and related issues.

All of your and your dog’s information will be kept private by your behavior consultant. In some cases, however, it can be beneficial for select information to be shared with other professionals.

Do you consent to your behavior consultant sharing the outcome of your consultation with your veterinarian? Such information will include your and your dog’s name, the particular issue in question (e.g. aggression towards strangers) as well as the advice given and the progress your dog is making, if you choose to follow through with the behavior modification plan created by your behavior consultant.
If your veterinary staff knows how to best treat your dog, they can be a valuable help whenver your dog needs medical attention. 

Do you consent to your behavior consultant sharing the details of your case anonymously with other dog behavior professionals, to receive advice on the treatment of your dog’s problem behavior?

Should your behavior consultant decide to share your dog’s case with other professionals, all identifying details (e.g. name, address, etc) will be omitted.
