    PGC-HS Certification Application Form

  • Please complete the following form to apply for Peer Group Connection (PGC) certification for your school. If approved, you will be sent certificates for your school site, eligible stakeholder team members, and eligible peer leaders. If we do not receive your application by Friday, May 12th, we cannot guarantee you will receive your certificates prior to the end of the school year.

  • Please enter the following information for any peer leaders who meet the requirements for PGC peer leader certification. Only peer leaders who meet all of the listed requirements are eligible.

    Requirements include:

    1. Adhered to PGC Code of Ethics
    2. Earned a B or higher in the course
    3. Attended the summer peer leadership retreat
    4. Completed at least 2 of the following:
      • Attended 90% of peer leadership class,
      • Attended 90% of outreaches,
      • Completed 90% of homework.
