Please use this form to complete your required 3-day food log. The importance of this log is it allows me to see what types of foods you're currently eating, assess your caloric and nutrient intake, and get an idea of changes we can begin making when it comes to your diet. Please be very thorough when completing this log. Complete all areas (breakfast, lunch, dinner, snacks, etc). List all amounts, brand of food, and type. For example, if you ate peanut butter you would list 2 tbsp Jiffy peanut butter. You would NOT list peanut butter. If you do not know the amount, eyeballing is okay. The more comprehensive your log is, the better information I can provide to you!
