2019 ADVISING AWARDS NOMINATION FORM
Complete the information below to nominate someone for a 2019 Advising Award. All winners and nominees will be acknowledged at the UCAADA End of the Year Banquet on May 8th. Note: Student nominations are only accepted for the Sarah Grant Barber Awards.
The nominations will close Wednesday, March 27th, 2019 at 5pm.
The Sarah Grant Barber Outstanding Advising Awards are granted annually to a professional advisor, faculty advisor, advising administrator, and advising support staff. These awards are sponsored by UCAADA and the Office of Advising & Academic Services. **Self-nominations will be accepted for the Sarah Grant Barber Outstanding Advising award** Professional, Faculty and Administrator winners will be UC's nominations for the NACADA Annual Awards, if eligible and interested.
Sponsored by the Office of Advising & Academic Services, the Advising Community Awards are open to those who play a role in the realm of academic advising, within seven different categories. Please note, student nominations are not accepted for the Advising Community Awards.
The Advising Unit of the Year award will be given to the Advising Unit that has shown excellence in advising and made significant contributions to the UC Advising Community. Units are only able to self-nominate. Nominated Units primary focus must be in Academic Advising and must include a nomination letter from the Director or higher.
Each nomination must include at least one letter of support, telling the awards committee how the nominee meets the award criteria (Self-nominees must include a letter from thier supervisor).
The initial nomination letter must be 12pt Times New Roman font double-spaced with one inch margins and limited to 2 (two) pages.
Additional letters of support are accepted - each letter should be formatted as above and limted to 1 (one) page for each letter. There is no limit to the additional 1 (one) page support letters.
