All information is kept and used in accordance with the Data Protection Act. The information held on any person can be checked by that person or their parent at any time on request.

As the parent or legal guardian of the above-named participant, I hereby give my consent for emergency medical care prescribed by a Doctor, Dentist or the Emergency Services. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent.
