This form is to be filled out by the Employee's Pastor or Supervisor (only with pastor approval).
No employee is allowed to begin working until the supervisor has been given the approval from Human Resources.
Annual Salary (must be $455/week or more, the employee must be full time and meet the FLSA Duties Test to qualify as a salaried employee)
Please Note: CTK staff are paid on a bi-weekly pay schedule of 26 pay periods per year.
Allowances Are An Addition To An Employees Regular Pay & Are Considered Compensation. These Allowances Are Paid On A Bi-Weekly Basis Of 26 Pay Periods Per Year & Show On The Employees W-2 At Year-End As Total Salaries & Wages.
Each Allowance Is Shown On A Separate Line Item On The Month End Reports Under Salaries & Wages & Can Be Budgeted For.
Allowances Plus Regular Pay Would Be The Total Compensation Package For An Employee.
