This questionnaire is designed to help you be as specific as possible with your information about the type of travel you're interested in, when you want to travel, and where you want to travel. The more specific you are about your travel wants, the better we can plan for your trip.

The trip planning service provided by Cruise Planners-Arizona Cruise and Travel is FREE. We are compensated by the cruise lines, tour operators, resorts, and hotels that you select for your trip.

We do reserve the right to charge a service fee for our time involved with planning your trip under certain circumstances. The primary circumstance in which we would charge a travel planning fee is if you completely change the itinerary of your travel. (i.e. After completing this form you ask us to plan a Carribean cruise but during the research phase you tell us that you want a vacation to Hawaii instead.) This type of change would require a Travel Planning Fee.

Travel Planning Fee Schedule

US Domestic Travel: $50

International Travel: $100

The fee will be charged before we begin researching the alternate travel destination and is non-refundable.

Additional Fee's

Airline Ticketing:

$35 (Domestic flights and Coach) per ticket

$100 (International flights and/or Business/First Class) per ticket

(This applies for the outbound and return flight. Should we need to purchase seperate intra-country air flights then the fee below will be added for those additional legs of the flight.)

$50 Intra-country per ticket.

Our airline ticketing fees are Non-Refundable under any circumstance. Should the airline request our fee back in order to refund a cancelled ticket then you agree to reimburse us for our fee.

By submitting this form I agree that I have done my best to describe the trip that I want to take. I have made my best effort to provide accurate spellings of names and accurate dates of birth for myself and other travelers. I acknowledge that it is my responsibility to confirm the accuracy of these passenger details when the final booking is completed and to notify Cruise Planners-Arizona Cruise and Travel of any corrections promptly. I also agree to the additional fees, as outlined above, should I use your travel agency to secure my flights for my trip.
