  • You have been asked to submit this questionnaire as part of the screening process for enrollment into research looking at the effect of nutrients on the gut. Your responses in this questionnaire will give a score that may exclude you from our research.

    Once we have received and scored the questionnaire, we will let you know whether we can proceed with the screening process.

  • 1. When I smell a sizzling steak or see a juicy piece of meat, I find it very difficult to keep from eating, even if I have just finished a meal.*
