I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED ON SAID ACTIVITIES, INCLUDING ACTIVITIES PRELIMINARY AND SUBSEQUENT THERETO. I do hereby agree to hold Landmark Church and its pastors, officers, agents, employees, bus workers, bus drivers, Sunday School teachers, and volunteers harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which I now have or which may arise in the future, in connection with participation in the described activity or in any other associated activities.
In the event any disagreement or conflict arises from any activity connected to this authorization and release, I knowingly and expressly agree to have such conflict resolved in accordance with the alternative Dispute Resolution procedures developed by Landmark Church, and agree to binding arbitration, expressly waiving any and all rights in law and equity to bring any civil disagreement before a court of law.
I give Landmark Free Will Baptist Church my permission to obtain emergency medical attention for the child named above. I understand that emergency transportation will normally be by ambulance and that I am to be billed for this service.

ENTIENDO Y POR LA PRESENTE ACEPTO ASUMIR TODOS LOS RIESGOS QUE PUEDAN ENCONTRARSE EN DICHA ACTIVIDAD, INCLUIDAS LAS ACTIVIDADES PRELIMINARES Y POSTERIORES. Por la presente accedo a mantener a Landmark Church y sus pastores, oficiales, agentes, empleados, trabajadores de autobuses, conductores de autobuses, maestros de escuela dominical y voluntarios exentos de cualquier y toda responsabilidad, acción, causa de acciones, reclamaciones, gastos y daños en cuenta de lesiones a mi hijo o propiedad, incluso lesiones que causan la muerte, que ahora tengo o que pueden surgir en el futuro, en relación con la participación en la actividad descrita o en cualquier otra actividad asociada.
En caso de que surja cualquier desacuerdo o conflicto de cualquier actividad relacionada con esta autorización y publicación, a sabiendas y expresamente acepto resolver dicho conflicto de conformidad con los procedimientos alternativos de Solución de Controversias desarrollados por Landmark Church, y acepto un arbitraje vinculante, renunciando expresamente a cualquier y todos los derechos en ley y equidad para presentar cualquier desacuerdo civil ante un tribunal de justicia.
Doy permiso a Landmark Free Will Baptist Church para obtener atención médica de emergencia para el niño mencionado anteriormente. Entiendo que el transporte de emergencia normalmente será en ambulancia y que se me cobrará por este servicio.
