This agreement documents the transfer of the dog named

The agreement shall remain in effect for the life of the dog or until the return of the dog to Paw Pals of Hale Center or a receiving rescue in the area.
An adoption fee of

will be collected from the Adopting Family at the time of adoption. The microchip will have the new adopter's phone number and name added as contact information. The chip is fully registered and active. (Type in "$500" in the box).
Paw Pals of Hale Center...I warrant that I am the lawful owner of the dog, able to convey the ownership of this dog to another home. Adopting Family... I agree that I will care for this dog in a safe and humane manner, and as a family pet and companion. The dog will have appropriate food, water, shelter, and medical care for the duration of his/her life.I understand that Paw Pals of Hale Center makes no guarantees or warranties regarding the health or temperament of this dog. The dog has been seen by a veterinarian prior to transport and has passed all necessary diagnostic tests and has received a certificate of health. Paw Pals of Hale Center shall not be held liable for the behavior of this dog or any damages it may cause. Paw Pals of Hale Center shall not be held responsible for any veterinary costs associated with this dog once it leaves their care, either as an adoption trial or a finalized adoption.I understand that if the dog is too young to have had the full 4 sets of puppy vaccinations at the time of adoption that it is my contractual obligation to finish the vaccination schedule as recommended by my veterinarian at my expense. I enter into this agreement of my own free will and understand that this is a binding contract enforceable by civil law. As the adopter, I understand my adoption fee is a non-refundable donation and that as the adopter I am required to keep the dog safe either in my residence or at my expense until another adoption of the dog can be completed or until transport with Project Freedom Ride can be arranged should the adoption not work out.
