PLEASE READ AND ACCEPT THE TERMS OF THE FOLLOWING STATEMENTS
BOOKINGS: To secure a date, a signed bridal contract and $50.00 deposit are required. The deposit is non‐refundable and non‐transferable. This deposit will be put toward the client’s event trial cost, if the client chooses wedding day services. The remaining balance will be due one week before the day of the event. Accepted forms of payment include: cash, check,Venmo and all major credit cards. Gratuity is never expected but always appreciated day of service(s).
We kindly request that each client provide electronic photos of each person worked on. They can be emailed to clevelandglamsquad@gmail.com
In the event that the contracted Makeup Artist/Hair stylist is unable to perform the services agreed upon due to an emergency, illness, or unexpected occurrence a trusted substitute Makeup Artist/ Hair stylist will be assigned and informed of the contracted and discussed arrangements. We do not accept last minute services on the day of the event. Please make every person that needs service is listed on the contract. We need a certain amount of time per person to give quality service.
DELAYS: A late fee of $30.00 will be charged for every 30 minutes of delay when a client is late for a sche duled time or if scheduled makeup exceeds allotted time because of client delays.
SERVICE LOCATION AND REQUIREMENTS: Please have a clean washed face. If hair service required please have hair washed and dried (extra services fee required). Ensure location temperature is cool and not hot so that makeup and hair will set properly. Ample lighting is also required, whether naturally or by lamps to provide the needed services properly.
EARLY CALL FEE: Any booked appointment times before 8am will incur an additional fee of $25 for each required member of the Glam Squad on site.
TRAVEL FEE: Travel within 10 miles of Cleveland, Ohio is 44113 completely free of charge. A mileage fee ($0.50/mile both ways)per member of the Glam Squad will be charged for locations outside of the 10-mile radius of Cleveland, Ohio 44113.
PARKING FEE: Any parking, valet or toll fees that may be incurred by the Glam Squad, must be paid by client on the day of the event.
LIABILITY: All brushes, tools, and makeup products are sanitized between every makeup application and are kept sanitary. Any skin conditions allergies or sensitivities of the client(s) should be reported to the makeup artist prior to application, and if needed a sample test of makeup may be performed on skin. Client(s) agree to release the makeup artist(s) (Cleveland Glam Squad LLC) from liability for any skin complications due to allergic reactions.
PAYMENT: The final balance is due one week before the day of the event No exceptions. The person(s) responsible for the entire balance of payment is the person(s) whose name(s) appear on this contract.Accepted forms of payment include: cash, check,Venmo and all major credit cards.
CANCELLATION POLICY: Cancellations must be made at least thirty (30) days prior to the client’s reserved date or the client will be responsible for paying the full amount of services agreed upon in this contract.
WE HAVE THE RIGHT TO REFUSE SERVICE TO ANYONE: If our makeup artist(s) or hairstylist(s) feel uncomfortable, threatened or verbally abused in any way, she will have the right to leave the premises without completing services.
