Please note, the form will not save in-between sessions, so it will need to be completed all at once.Applicants for the 1st and 2nd grade classes of 2019-2020 will be assigned an assessment time as soon as there are enough applicants to fill an assessment session. The application is due January 15, 2019. All assessments for 1st and 2nd grade will tentatively be held in late February and March 2019.

Please note that no child can be assessed before their 4th birthday. Also, in instances when twins/triplets are applying, it is our experience that twins/etc. do not perform well when in the same assessment group. Please schedule siblings in different time slots (they may be the same day).
