    • Please note that to preserve uniformity and fair marking, the entire entry should be submitted on this form only. Please stick to given guidelines or entry will be disqualified. Addendums are not permitted.
    • Please note that the entry should have been implemented and/or commercialized between April ‘18 and March ‘19.
    • Individuals or teams can apply for the award (you can submit yourself or your team as well).
    • Supervisors or senior management can apply on behalf of a team/individual.
