Please complete the required information 6 weeks prior to the request OFX Day to allow for processing and approval time.
Prior to running any paid ads to promote your OFX Day, please provide:
1. Photos of the doctor and practice (ideally featuring OFX)
2. Practice commitment to post content on their FB Page promoting OFX Day
3. Optional- brief video of the doctor talking about their OFX Day excitement
