1. Following from the signing and return of this Customer Activation Form, we shall provide GPS Tech with a reasonable time, date and location for the installation of the tracking hardware in the above vehicles.
2. We shall pay TWO (2) months rental as deposit prior to installation for tracking hardware & the installation charge of RM200.00 per vehicle upon 1st billing.
3. Also, within fourteen (14) working days from the installation of the tracking hardware in the above mentioned vehicles, we shall arrange for a reasonable date, time and venue for GPS Tech to conduct training for us with respect to the proper utilisation and maintenance of the tracking services provided by GPS Tech.
4. Kindly bill us in accordance with the terms of the Quotation.
Consent Authorization under Personal Data Protection Act 2010 (“PDPA Act”) and Credit Reporting Agency Act 2010 (“CRA Act”)
By submitting this Form, you hereby agree that the Company may collect, obtain, store and process your personal data that you provide in this form for the purpose of providing / supplying our services / goods to you.
You hereby give your consent to the Company to:-
a. Store and process your Personal Data;
b. Disclose your Personal Data to the related governmental authorities or third parties where required by law or for legal purposes.
For the purpose of updating or correcting such data, you may at any time apply to the Company to have access to your personal data which are stored by the Company.
For the avoidance of doubt, Personal Data includes all data defined within the Personal Data Protection Act 2010 including but not limited to all data you had disclosed to the Company in this Form.
Apart from the above, pursuant to the CRA Act you hereby agree to allow CTOS Data Systems Sdn Bhd, a registered credit reporting agency under the CRA Act to process your company and personal data as per PDPA Act
