This application is for financial assistance in the case of a pet needing extensive veterinary treatment for a diagnosed disease or injury. DOGS MUST BE CURRENT ON BOTH RABIES & DHHP OR DHLP. CATS MUST BE CURRENT ON BOTH RABIES & FVRCP. YOU MUST RESIDE IN MAHONING,TRUMBULL,COLUMBIANA,PORTAGE OR STARK COUNTY TO APPLY.

The following documentation is required for consideration:

1) Proof of financial hardship, or SSI or other government financial assistance with your name on it. (WIC, EBT, etc)

If not on assistance please provide most recent paystub. ( If married, both stubs).

2) Driver’s license or state identification

3) Proof of ownership. (License,current Rabies cert.or copy of vet bill)

4) Proof of CareCredit denial or approval.

5) An estimate generated by a veterinary clinic for the proposed treatment plan

We will respond to your completed application, submitted with all required documents, within 24-48 hours. Due to the large volume of applications, we will not respond until all documents are received. 

Approval for funding will be determined at the discretion of the board members of Busters’ Brigade. Please note that Buster’s Brigade will not fund 100% of the estimate and that the percentage or dollar amount will be determined by the board members.
