Third Year Parent - Subject Teacher Meeting Booking Form

• Starts at 4:15pm.
• Ends at 6:45pm.
• All meetings will take place in school hall.
• Given the limits of time, we estimate that to make the best use of the slots available, you should focus on trying to see a maximum of seven teachers.
• Missed meetings cannot be rescheduled for later that evening, as this would upset the schedule for other parents/guardians/carers.
• Please arrive and depart on-time for all meetings.
• Meetings must be reserved using this form and are not possible without an appointment.
• Each meeting will last 5 minutes only.
• Our PSA (Parent Staff Association) will be selling refreshments during the meetings. Please allow some time for a break and an opportunity to meet PSA members.

Please ensure that you leave enough time between meetings to find your next meeting.

• Please discuss with your child, which teachers might be a priority to meet. Click on seven teachers' names on the next page. This will create seven appointment pages which will pop-up on the next page.
• Reserve a meeting-slot with these teachers on the next page.
• Once you have made your choices, click SUBMIT. You will then receive an email detailing your selections.
• Print this email and bring it on the night of the meeting as a helpful reminder.

Be careful not to select the same time for two different meetings as this system will not be able to warn you if you do so.

Please tick a time that you would like to meet a teacher. Ensure that you don't book the same time twice and that you leave time to travel between teachers. If a teacher is full, you can always press 'Back' and select a different teacher. Please click Submit, when you are finished.
