2023 ACTIVITY RENTAL FORM

ONLY the lead person named on the booking needs to complete this form as a point of contact for your group.

You will need to complete a form each time you come to ensure we have the correct data recorded in case of an incident/accident for the day you attended.

It will take a few minutes to work through the form and to AGREE OUR RULES & RECOMMENDATIONS for use of the facilities.

Completion and signing this form is a CONDITION OF USE and must be agreed prior to rental commencing.

If you cannot agree to the RULES & RECOMMENDATIONS then you must abandon your application and contact us using office@activitiesaway.uk to cancel your booking.

PLEASE NOTE - you may experience some 'lag' while completing the form, this is normal as our database is updating in the background as you complete the form and is affected by things like number of users and the available bandwith or speed of your internet connection.

If you just wait a few seconds it should catch up and you will be able to complete the process.

If you have any questions at any point during the application then you must abandon your application and email us using office@activitiesaway.uk or call us on 01522 697527 to clarify any points before you apply to be a member.

We keep the collection of your personal data to a minimum and we only ask you for data that we require to determine your application to use our facilties.

We are the DATA CONTROLLER

JOTFORM Inc. is the DATA PROCESSOR

We only store your data in an ONLINE DATABASE that is GDPR compliant and is controlled by JOTFORM who process our forms for us.

We do not print or move your data and ensure that only staff who require access to it to determine your application, to check you are a member or in an emergency are able to view your data.

We will remove your data within 30 days of participation or sooner if you notify us in person, by email, telephone call or post that you no longer wish to use our facilities.

We reserve the right to retain your data in the case of non payment or for pursuing a damage claim until payment has been received and/or damage claims settled.

Once removed you will not be able to use our facilities unless you complete another form prior to rental.

1, I acknowledge that I need to PRE BOOK via the website or phone PRIOR to attending for any rentals/activities
2, I acknowledge that I may be refused entry if I attend without PRE BOOKING ACTIVITIES
3, I acknowledge that I have read and understood any information provided to me by the centre prior to arrival.
4, I acknowledge that I am over 16 and understand that any participants under 16 must be supervised on the water by a responsible person over 16 years of age unless otherwise agreed at the time of booking by contacting the office.
5, I acknowledge that I must wear the safety equipment provided at all times and refusal to wear the safety equipment or removal of the equipment while on the water will terminate your session with no refunds offered.
6, Participants must stop and render assistance to any other participant in difficulty.
7, If you are in difficulty make your way to the nearest bank and wave your arms if possible or exit the lake at the identified exits and return to the centre and make staff aware.
8, All equipment used must be returned to the designated area by users at the end of the rental period and any damage reported to staff.
9, If you are a nervous participant we recommend staying at the centre end of the lake.
10, Please advise staff of any changes in your personal details on arrival including any changes in your medical circumstances that may affect your participation on the day.
11, Your rental starts at the stated time - we will try to accommodate you if you are running late but this is not always possible and you may lose rental time.
12, Rentals finish at the stated time - all participants must be out of the water by this time.
13, No rental will commence unless staff have given permission and you have signed in.
14, Participants must sign in and payment must have been received prior to commencing activities.
15, Any deliberate or significant damage is the responsibility of the group leader signing this form and must be reported to staff on the day and paid for prior to departure.
