Important Insurance Coverage Information For In Network COVID-19 Testing
Under Federal Law insurers are required to cover Covid-19 Testing and a related visit if :
(1) You are experiencing symptoms consistent with Covid-19. Such symptoms include, but are not limited to - Fever or chills, Cough, Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat, Congestion or runny nose, Nausea or vomiting, Diarrhea; or
(2) You have reasonable grounds to believe you may have been exposed to a person or persons with confirmed or suspected Covid-19.
If you are not experiencing symptoms and have not been exposed to the Covid-19 virus, your insurance may or may not cover your testing (for example, if the only reason for your test is to travel somewhere, or for return to work or school, your insurance is not likely to cover the test).
In that situation :
(1) CareCube will determine whether your insurer has advised us that the test is not covered; and
(2) If your insurance has advised us that the testing is not a covered service, you will be required to pay the full costs related to testing up front.
(3) If your insurance has advised us that the costs related to such testing are covered services, you will not be required to pay anything up front.
(4) If your insurance has advised us that the costs related to such testing are covered services, but a co-payment, co-insurance or deductible is required we will collect that amount up front.
(5) Finally, if we are uncertain whether or not your insurance will cover the costs of testing, we will not collect any upfront fee, co-payment, co-insurance or deductible from you at the time of service, and CareCube will submit a claim for reimbursement to your insurer. Then:
- If your insurer covers the services provided without requiring any co-payment, co-insurance or deductible, CareCube will not bill You.
- If your insurer denies coverage because the services are not covered services CareCube may bill you the amount listed on the attached pricing sheet.
- If your insurer advises CareCube that a co-payment, co-insurance or deductible must be collected from you, CareCube may bill you that amount.
