Family Programs Sign-up:
SIGN US UP FOR FAMILY DINNERS! Every other month, join a Zoom with 2-3 other families. Eat dinner together, chat, and/or play a game. Each month, different families will be grouped together. It's a fun, low-key way to connect with church friends. Meets Sep, Nov, Jan, Mar, May.
WE'D LIKE TO BE A CHALICE LIGHTERS HOUSEHOLD: Join the rotation of households who light the Sunday service chalice. All Zoom technical support provided, it's not hard to do.
SIGN US UP FOR THE MONTHLY "CREATIVITY PROJECT" GROUP: Come together to make a craft project related to the worship theme. Easiest for Grades 5+, kids must attend with family. You will bring your own supplies from a provided list. Usually 2nd Friday evenings/Saturday mornings, tbd.

Gr.6-12 use group emails and texts for RE group coordination. UUSM follows the safety rule that 3 or more people must be on all youth communications (never 1-1), and contact info is only used for official RE-related items, not personal conversations.

CHALICE CHILDREN (PreK-Gr.2): 2nd Saturday 9:30-10am
EXPLORERS (Gr.3-5): 1st/3rd Sundays, 12 noon -1pm
TREKKERS (Gr.6-8): 1st/3rd Sundays, 4:00-5:00pm
YRUU YOUTH GROUP (Gr.9-12): 1st/3rd Sundays 5:30-6:45pm
Other programs my child/youth wants to participate in this year:
YOUTH JUSTICE PROJECTS (Gr.3-5/Gr.6-10): Times TBD, estimating no more than a few hours a month on average.
SUNDAY SERVICE PROJECTS (All ages): Record songs, do a reading or skit, create art and other projects to be shown in Sunday service. Kids/youth can choose what they will participate in, and if solo or group.

We are asking every family to help in some way. Many hands make more fun, and we can't make the magic happen without you!

TEACHING TEAM: Leads will do 2-3 months (two 1-hr sessions/month) , then rotate off for one month, etc. Other schedules possible. It would be best to have 2 leads at a time. (The new whole-church curricula has very minimal setup, and DRE will do most/all prep. Zoom training is provided and it's not difficult!)
RE "ROOM PARENT": Help DRE with encouraging families in your child's RE class, provide feedback, help coordinate occasional activities or tasks if needed, all-around cheerleader and good egg.
RE SECOND ADULT: Attend classes/sessions twice a month, assist the Lead Adult and be the second adult required by the safety policy.
HELP WITH FAMILY PROJECTS/ACTIVITIES: Coordinate and help with ongoing activities for families.
CONTENT UPDATES: Help maintain the Home Activities Padlet webpages (approx 2 hrs/month)

Medical Information and Waiver
As we are online this year and guardians are responsible for their child(ren) at home even during UUSM online meetings and classes, we will not collect all the usual medical information at this time. When we are able to begin doing some safe socially-distanced events, we will send out "field trip" forms for this information. Please make sure the Concerns field below includes everything Zoom leads should be aware of during Zoom/ online meetings.

UUSM may photograph and record my Youth’s likeness and activities (pictures, images, audio or video) during UUSM-related activities. UUSM has permission to use and re-use, publish and re-publish and modify or alter the pictures, images(s), audio and video, for materials related to UUSM. We release and discharge UUSM from any and all claims arising out of use of the pictures or images for the purposes described above, including any claims for libel, right of publicity, or invasion of privacy.

I hereby give permission for my child/youth to attend and participate in any online UUSM child/youth activity. I understand it is my sole responsibility for my child/youth's safety during any online UUSM activity, wherever they are located physically. I further understand that any later transition towards in-person gatherings will be discussed and approved by parents for their child before being implemented.
In consideration of UUSM allowing your child/youth to participate in child/youth activities, we the undersigned, on behalf of ourselves and the child/youth, do hereby release, forever discharge, and agree to hold harmless UUSM, its ministers, employees, officers, volunteers and teachers (“UUSM”) from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, or any nature whatsoever which may be incurred by the undersigned and/or the child/youth while involved in child/youth or other church activities. We, the parents or legal guardians of this child/youth hereby grant our permission for the child/youth to participate fully in child/youth activities. Furthermore, we, on behalf of our minor child/youth, hereby assume all risk involved therein. The undersigned further hereby agree to hold harmless and indemnify UUSM for any liability sustained by UUSM as the result of the negligent, willful or intentional acts of said child/youth, including expenses incurred attendant thereto.
